How do you get started?

There are three simple steps to join. Step #1 fill out the Nutrition Assessment
form. Step #2 Fill out the 24-hour recall form. Step #3 go to order form
for payment options.
Once we receive your Nutrition Assessment and
24-hour recall, we will send your welcome package via priority mail within one week. Your welcome package will include
our Diet Exchange
Kit™ also known as the Counter*, 14 Daily Food & Exercise Diary sheets for you to learn to monitor your food intake, your personalized nutrition plan, and an analysis of your current food and exercise habits.
The Counter is a small hand held device that helps you keep track of how much of each food group you are consuming throughout the day and how much is left. It was designed small enough so that you can have it at all times. It can fit into a suit jacket, a small pocket book, or even a day timer. It is not to give you the feeling you are on a diet but rather increase your awareness of what and how much you are eating. It is a necessary tool for
today's busy world where people are just not making time to think about what they are eating. The
"Counter" will help put the nutrition plan provided into action. You will also be sent some initial suggestions and tips to help you improve your current eating habits and food choices. Just think, it has taken all these years to develop the habits you have, it will take some time and work to incorporate some of these life lasting changes.
When completing the nutrition assessment form, be sure to schedule your block of time for your weekly one on one 15 minute conference sessions with your nutritionist. We will assign you your own personal password. This is exclusive for club members. Your "chat" will begin Week 2.
What
happens next?
After the first week of using the meal plan and putting
our suggestion to work on your current eating habits and food choices, you will
have three more “visits”.

with additional help, suggestions, and encouragement.
If needed, adjustments to the meal plan will be also given (there will be
meal plan cards provided to change the plan if necessary).

Week 3

return to the "Follow up" form, so we can monitor the changes and progress you are making.
At that time we will ask you to weigh yourself again (if appropriate).

Again, a response will be provided
letting with provided more guidance and encouragement.

Your last but not least "Follow up" form should be
submitted. You
will be asked to weigh yourself again (if appropriate)

The fee for continuing your enrollment in the Home
Nutrition Club will be $75.00 per month. First, complete your follow-up form that will be forwarded to your personal nutritionist, via e-mail. Make your appointment with your nutritionist who will discuss your progress during your weekly 15-minute private chat. As always, our discussion groups, and e-mail are always available to you.

Whether you are a current member of The Home Nutrition Club or an owner of The Diet Exchange Kit, join our Maintenance
program.
For only $50.00 per month you will fill out our Follow-up form, twice monthly, which will be forwarded to your personal nutritionist. You will then have your appointment for your 15 minute, one-on-one private chat with your nutritionist. This is to discuss your progress, any problems or concerns you might have, or just to let us know how you are doing. As always, our discussion groups and e-mail are always available to you.
